Who we are

This website belongs to Dr. Akshada Health Care Clinic, operating from Kedarnath General Hospital, Vishrantwadi, Pune, Maharashtra 411015, India. Dr. Akshada Chavan is responsible for the information collected here.

What we collect

We only collect what you type into a form. Nothing else on this site tracks you: there are no analytics, no advertising tags and no cookies set by us.

• Contact form — your name, phone number, subject and message.
• Appointment form — your name, phone number, gender, date of birth, address, preferred date, and the reason for your visit, which may describe your symptoms or health condition.

Why we collect it

To answer your question, to confirm and prepare for your appointment, and to keep a record of your care. We do not use it for marketing, and we do not sell or share it with anyone for advertising.

How it reaches us

Form submissions are delivered by e-mail through Web3Forms, a third-party form-delivery service that processes the submission on our behalf and passes it to the clinic's mailbox. The connection is encrypted in transit. Web3Forms also uses hCaptcha to tell real visitors from automated abuse; hCaptcha may process your IP address and browser information for that purpose.

Fonts and images on this site are served from our own domain, so simply browsing these pages does not report your visit to any third party.

How long we keep it

Appointment and consultation records are retained for as long as required for your continuing care and by the record-keeping rules that apply to medical practitioners in India. General enquiries that do not lead to an appointment are deleted once the question has been answered.

Your choices

You may ask us what information we hold about you, ask us to correct it, or ask us to delete anything we are not required to keep. Write to dr.akshada.chavan@gmail.com or call +91 83298 99809 and we will respond as soon as we reasonably can.

You are never obliged to use these forms. If you would rather not send your details over the internet, please call the clinic and we will take your booking over the phone.
